1. Don’t register, if you are not ready to pay ($110.00)! Once payment has been made a refund is not available!
2. If you are an employer paying for your employee please list that employee’s name in the shipping information when making payment to avoid delays with account access.
3. Fill out the registration form.
4. Make sure you spell your first and last name right (“John Do” not “jonh do”)!
5. After click “submit”, you will be redirected to the payment page.
6. Within 2 days you will get access to your account and materials, which you will be informed about by email. If you didn’t receive an email try logging in using your username and password. 7. After login-in click in “My account” then select “QMAP Online Corse” to the top-right menu.
8. Participate in the self-study materials and skill guide.
9. During 14 day access, you have to schedule an exam or you must register and pay again The exam is held in-person at our school. Click here to see our school address.
10. After choosing a date of exam, click submit.
11. In the exam, photo-ID and SSN is required.
12. (I agree) If a student fails the competency evaluation or the final exam, they must register again and repeat the entire course.
13. The student understands they have 2 weeks (14 calendar days) from the date access is received to complete the course material. On day 15 ALL account access will be closed. If you have not completed the training within the 14 days, all monies paid will be forfeited, thus you will need to re-register and pay again.
